E-1.Before the visit

"I've been having symptoms I think are related to trauma. Since ___, I've had [nightmares / trouble sleeping / feeling on edge / avoiding things]. It's affecting [work / my family / my sleep]. I'd like to be screened for PTSD and talk about treatment options, including therapy referrals."
